I thought Ellie was a doctor. A hyperinflated scrotum could be an emergency and needed draining. Perhaps the joke could be that Ellie says something like "No need to go to the ER for this. You could just have hired a hooker!" and Zarah gets offended he didn't hire her tushy instead.
